COVID-19 Vaccinations and Boosters Available on Campus

The COVID-19 vaccine is the single most effective way to protect our community. All BW students, faculty and staff are eligible to receive a free COVID-19 vaccine at the BW Health Center or at retailers within walking distance from campus.

To schedule a COVID-19 booster vaccine appointment, call the BW Health Center at (440) 826-2178. Bring your picture ID for vaccinations and boosters and your COVID-19 Vaccination Record Card for boosters. Please continue to read details below before calling to schedule.


About COVID-19 Vaccinations and Boosters Available on Campus

BW offers the Moderna vaccine and booster and the J&J Janssen vaccine. View information about the Moderna vaccine (2 shots, 1 month apart) and the J&J vaccine (1 shot). Individuals who want to receive the Pfizer vaccine or booster should contact other vaccine providers. Current CDC guidelines consider a person fully vaccinated two weeks after completing the vaccine.

Providing Record of COVID-19 Vaccination

Based on guidance from the CDC and the Ohio Department of Health, non-symptomatic students, faculty or staff who have received a complete COVID-19 vaccine will not be required to quarantine if they are an identified close contact of a positive individual and provide documentation to support this. Vaccinated students, faculty and staff can submit proof of COVID-19 immunization(s), including booster information, through the Med+Proctor system which BW uses to track and provide compliance, using their BW email address.

Note: If you submitted your vaccination information to BW Health Services during the spring 2021 semester or are a new student who submitted vaccine information via onboarding forms through BW's Admissions portal, you MUST also upload your information to Med+Proctor. This is the system the University will use going forward to track vaccinations and waive quarantine and surveillance testing protocols.

All BW students, faculty and staff can provide evidence of vaccinations by uploading a photo of their vaccination card through the secure platform at Med+Proctor.

There is no charge for this service, though you have the option to purchase a membership that will provide lifetime access to your immunization data that can be shared with others, as you designate. (Please note: You do NOT need to purchase the membership or submit a doctor-signed immunization certificate.)

Steps to Submit Proof of COVID-19 Immunization

  • Log in to Med+Proctor using your BW username and password.
    • Note: If you have not previously created an account, click the link above and enter your BW email address. Instructions to register will then display on your screen.
  • At the bottom of the screen, click on "Click here to submit another document"
    • Note: This action works for submitting 1st and 2nd doses as well as booster information.
  • In the "Document Type" drop-down menu, select "COVID Documentation: Optional."
  • Click “Choose file” and navigate to your vaccination card photo.
  • Click "Continue."
  • You will receive a confirmation email when your information has been verified by Med+Proctor.

Questions and Support

If you have difficulty using Med+Proctor, you may contact Med+Proctor between 8 a.m. and 5 p.m. Monday through Friday. During those hours, you can live chat with their support agents directly from their support site or while you are completing your upload. This feature is located at the bottom right of the screen on either Med+Proctor site.

You may also email help@medproctor.com with your issue, and this will create a ticket for their support agents to take care of you.
