The following employment forms are provided for the convenience of newly hired full and part-time faculty and staff. Completing the forms prior to visiting campus will streamline the onboarding process and help with processes like setting up your BW email account, getting you on payroll, etc.

In addition to the forms listed above, all new hires are required to complete an I-9 form which must be done in person. In accordance with the Immigration Reform and Control Act of 1986, as amended, an offer of employment is contingent upon submitting certain original documents on or before a new hire's first day of employment to verify identity and employment eligibility. These requirements are typically met by presenting a driver's license and a Social Security Card or birth certificate, OR a valid passport, or other legally acceptable documentation.

As a reminder, all new hires must successfully complete and pass a background check. Procedures for conducting the background check are explained in offer of employment letters and employment contracts mailed to new hires.
