Important! Please read the following instructions:

  • Use the student's full name, rather than a nickname, in your nomination materials.
  • Complete all fields on the form.
  • All completed nominations must be received no later than 5:00 p.m. on Tuesday, February 26, 2013. Nominations submitted after this deadline cannot be considered for this year's award.
