Baldwin-Wallace College Health & Counseling Services provides comprehensive health care to students. To facilitate provision of care, it is helpful to have background medical history information and immunization records on new students. It is strongly recommended that each student complete the Health History/Record of Immunizations form and have a physical by your private physician on file at the Health Center. If you are an intercollegiate athlete, you are required to have a physical with B-W's Team Physician at no cost to you. Please be sure to complete the front portion of the form and send it to Student Health Services. Any information provided on the form is STRICTLY CONFIDENTIAL and is not given to anyone without permission of the student. For your convenience, we have made the form available on-line. It is also recommended that you participate in the Hepatitis B program offered by the Health Center. There is a series of three injections given to prevent Hepatitis B. The entire series of injections must be taken in order for the vaccine to be effective. We also recommend the Meningitis vaccine. If you are not already vaccination for Hepatitis B and Meningitis please review the following vaccine information and contact Health Services if you have questions.
