PLUS Loan Endorsers
The Endorser Addendum can be completed through the Department of Education's web site:
To complete an Endorser Addendum, you will need:
1) Approximately 20 minutes to complete the online form
- Endorser Addendum must be completed in a single session, be sure you allow enough time to complete.
2) Your Federal Student Aid PIN
(Endorsers must sign in using their own Federal Student Aid PIN to complete a PLUS Endorser Addendum)

- If you do not have a Federal Student Aid PIN please visit the PIN site.
3) A Loan Reference Number or Award ID
- The borrower will provide you with a loan reference number or an award identification number. Each loan you are endorsing will have a separate identifier.
4) Personal Information
- Permanent Address
- Mailing Address (if different from permanent address)
- Telephone Number
- Email Address
- Alien Registration Number (if you are an eligible non-citizen)
5) Employer Information
- Employer Name
- Employer Address
6) School Name
7) Reference Information for two people with different U.S. addresses who have known you for at least 3 years. Do not list the Direct PLUS Loan borrower or the dependent undergraduate student as a reference.
- Name of each reference
- Address of each reference
- Telephone number of each reference
- Relationship of each reference to the endorser
