- 08/09 Enrollment Worksheet for Independent Students
For independent undergraduate and graduate students who have applied for financial aid. This form must be complete before financial aid eligibility for independent undergraduate and graduate students can be determined.
- 08/09 Dependent/Independent Verification Worksheet
For students who have been selected for verification. Note: You should only complete this form if you have been specifically asked to do so by the Office of Financial Aid.
- 08/09 Ohio Choice Grant Eligibility Statement
For Ohio residents who do not intend to file a FAFSA but would are eligible to receive the Ohio Student Choice Grant.
- 08/09 Scholarship and Special Award Application
To be completed by students who would like to be considered for scholarships and special awards.
- Employee Tuition Remission Request Form
To be completed by employees of the College who are requesting tuition benefits at Baldwin-Wallace College.
- United Methodist Scholarship Application
To be completed by students who wish to be considered for the United Methodist Scholarship. Students must have at least a 3.0 GPA and must have been a member of the United Methodist Church for at least one year. *Note: You must also submit a Certification of Membership form, to be completed by your Pastor. - 08/09 Siblings in College Form
To be completed by those who have siblings attending another college on at least a half-time basis.
