Your application for the CSI Academy includes an application form (with an attached Student Essay) and a nomination form. All materials should be mailed together in one envelope.
- Complete the application form and essay. If possible, complete the form online and then print the form. If necessary, you may print the form and complete it by hand. Be sure to sign the form.
- Print the nomination form. Be sure to read and sign the FERPA statement.
- Deliver the nomination form to your science teacher. Ask him/her to place the completed nomination in a separate envelope, seal it, and sign it over the sealed flap before returning it to you.
- Place everything in one envelope and mail to the Academy Administrator.
- Do not send items separately. Do not send items electronically.
